[Qwest] Modem return issues

My parents have prism tv with 100mbps internet. When they signed up, they got modem rental free for 1 year. The year was up in september, and we returned their modem. (I had a spare c2100t) - it was received on August 1st. Now it's nearly november and i've had to call each month to get a credit and have them remove the modem off the account. They give me change numbers each time, but somehow they magically disappear. Yes, i've kept the return tracking number, so that's not the issue. But calling them every month is exhausting. Does anyone have any other ideas? Thanks!
