Century Link keeps charging me for equipment I don't have.

I reactivated my gigabit fiber line about 5 months ago. I already had everything in place from a previous install. Century link reactivated the port that my ONT is connected to, I called customer service and entered my PPOE credentials into my router and tagged for vlan 201. I was up and running in less than 5 min before the installer showed up. I tried to cancel the install, but Centurylink said they couldn't do that because it would cancel the port being turned up and the installer had to show up. When he did he was here for five min, I explained to him I was up and running, he checked his tablet and said ok and left. Next thing I know I'm being charged for an install fee and a modem rental charge. I call up customer service, they were able to research and remove both charges. Fast forward 3 months, now I'm being charged a unreturned broadband equipment charge for 108.09. It turns out the tech never marked on the work order that the equipment wasn't installed and just closed out the work order. I'm now being charged for equipment that I never got. On top of it, it's a nightmare trying to get anyone at Century Link to credit my account. I keep asking CL what the modem ID is that's on my account if I had gotten one, and no one can provide it. Well I know why because I never got one!! Super frustrated right now. Anyone have any insite what it takes to get this corrected for good. I'm tired of seeing charges pop up for a modem I don't have nor do I need.
